Biography
Dr. Wozniak joined the D’Youville University School of Pharmacy as a Clinical Assistant Professor in the Department of Pharmacy Practice in August of 2019. After earning her Doctor of Pharmacy degree from D’Youville University School of Pharmacy in 2008, she pursued a PGY1 Pharmacy Residency at Mercy Hospital of Buffalo. Upon completing her residency, she returned to D’Youville University School of Pharmacy, where she established a practice site in Ambulatory Care at D’Youville Health Professions Hub. Here, she developed numerous clinical services, including an ADCES accredited Diabetes Self-Management Education and Support Program. During her tenure as a Clinical Assistant Professor, Dr. Wozniak also served as Director of Interprofessional Education and as Residency Program Director for the D’Youville University School of Pharmacy/Vital Pharmacy’s PGY1 Community-Based Pharmacy Residency Program. In January 2024, she assumed the role of Assistant Dean of Clinical Affairs, Outreach, and Special Projects. In this position, she is charged with developing and implementing a strategic vision for the school's clinical services and outreach initiatives. This includes overseeing patient care provided through DYUSOP’s Clinical Health Services, the Hub, and related entities, enhancing relationships with healthcare facilities and preceptors, and securing funding opportunities to advance clinical services and the pharmacy profession both locally and internationally. She is also currently pursuing her Master's of Business Administration degree at D'Youville.
